In cases of pelvic contraction, where the true conjugate dimension is significantly reduced, it can be difficult to deliver the shoulders after reducing the size of the head.  In such cases, simple division of the clavicles (cleidotomy) can be performed to collapse the shoulders, thus facilitating delivery.  Similarly, they were employed to disarticulate the fetus (embryotomy) when required for the same reasons.
