When operating by this method some degree of prolapse is preferred.  In cases of full protrusion of the cervix, with or without modest traction, the patient is positioned on her back.  If the uterus is high up in the pelvis a lateral position and speculum is preferred.  The chain of the ecraseur is simply positioned about the base and the part should be slowly cut through.
