Brissez’s Intrauterine Specullum by Collin was used to dilate the os cervix.  Turning the thumb-wheel at the proximal end draws an internal wedge towards the handle and so separated the blades of the speculum.  The octagonal handle is of smooth ebony, the thumb-wheel of German Silver, the shaft of un-plated steel, and the speculum of brass.
