This form is illustrated Evans & Wormall’s 1889, Maw’s 1830 catalog and I have seen the same formed by Tiemann as well.  The pivot points are similar to other multi-valved forms but the manner of activation is by means of a gimbal-type arrangement joining the two anterior blades to one end of a handle with a pivot and spring lever arrangement secured by a thumb-wheel.
