Sims’ Pulley of Fork is designed to exert pressure on both sides of the needle point during fistula repair and to adjust the arms of silver sutures by providing a smooth fulcrum over which the suture could be drawn over to avoid tearing of delicate tissues or brought down upon a knot or lead shot.
